The Key to Keeping Patients at Home

Specialty services provided by expert clinicians address the specific needs of patients with a variety of diagnoses and needs such as diabetes, heart disease, stroke, cancer, dementia, wounds, incontinence, ostomies, joint replacements and mental/behavioral health problems.

Care of Chronically Ill Patients

  • The VNA is a Chronic Care Professional (CCP) accredited agency and all chronic care nurses hold Professional Certification issued by the Health Science Institute. Clinical Best Practices of Care include the use of evidence-based protocols, self-management support, motivational interviewing and health coaching.

Telehealth

  • Nursing visits are supplemented with daily monitoring of weight, blood pressure and oxygen levels using touch screen technology. Medical information is sent over the phone lines to the visiting nurse and physician who monitor the results daily.

In-Home Palliative Care

  • Provides care to patients with life-limiting illnesses at any stage of illness, whether patient is receiving curative treatments or considering hospice. Our goal is to alleviate suffering through pain control and symptom management measures to improve the patient’s quality of life.
